I was reading online reviews of The Wind That Shakes The Barley when I came accross the following from Ray Bennett's review in The Hollywood Reporter - "the Irish accents are so thick that even English subtitles are required."
What do US cinema goers require? That all movie characters, regardless of ethnicity, should speak with American accents? Mind boggling.

Of course. And they want to know if the film is worth watching in the first place, via previews that give the whole plot away. And it can't be more than 2.5 hours long. And the dialog to action ratio is very important. And the plot must not require too much brain power.
Mind you, these characteristics are not exclusive to the American movie goer.
